What to bring to your DOT Exam..

Please read below for many items that are required for your DOT exam. This will save you time!

If you have any questions regarding the items below, or are not sure if they pertain to you, please give us a call so we can help.

Identification

Current Driver's license/CDL/ID card & Current Medical Card (if applicable).

Medications

A list of the names and dosages of any Prescription or OTC medications you are taking. Please remember to take your medications as prescribed to ensure an accurate assessment.

Corrective Lenses/Hearing Aids

Bring your corrective glasses or contact lenses for your vision test, if applicable.

 

Bring your hearing aids for your hearing test, if applicable. 

CPAP Compliance Report

Drivers with Sleep Apnea need to bring their CPAP compliance reports for the past 30 and 90 day usage.

Cardiac Echo/Stress test reports

Drivers with Cardiovascular disease need be tested annually and bring the the most recent results to your exam.

Diabetes

Drivers need a Hemoglobin A1c blood test every 3 months - bring your most recent lab results to your exam. If you do not have a current test, we can provide one for an additional $25. 

Medication Release Form

Must be signed by your Primary Care Physician - Certain medications to treat conditions, such as ADD/ADHD, mental health disorders, sleep disorders, nerve or muscle disease, and others, require written authorization from your PCP before passing your DOT physical exam.

Blood Thinner Rx

Drivers need INR blood work every month - bring your most recent lab results to your exam.
